You can use this to link to a form to record your volunteer hours (not to be used when you are on Patrol). The section leader signs off the hours as approved. This replaces the hard copy system we used to run.

Lifeguard Uniform Request Form
Uniform only available to refreshed and financial members – This form is to request replacement of worn out / damaged uniform. Your request will be reviewed by the club and if approved, the uniform will be provided. If you are sitting your lifeguard award for the first time, it will be ordered for you by the club instructor, there is no need to fill in this form. Uniform only to be worn when on patrol, refer to the SLS NZ policy on uniform use.

This is not used for Surf Lifeguard Award, Patrol Support or IRB Driver Award – Those courses are run by East End in-house and registered for separately.
If you express an interest in a course and later wish to withdraw that, please email – hywel.eastend@gmail.com. If we cannot find a replacement and its a First Aid or another course that has a cost; you will be liable for the cost. The same applies for a no-show.
